Ms. Karolyn Lacey, Health Aide, works with the School Nurse. Medication Administration at School
Any Prescription or Over-the-Counter medication that is to be given to or taken by your student while at school or during a school-sponsored event/activity is required to have an Annual Medication Order & Parent/Guardian release/request on file, prior to the medication being taken. The medication order form is available below. Elements required are - Dr./Dentist order for the medication, Name of Medication, Dosage, Frequency, Reason for Medication, Signature of Dr./Dentist, and Parent/Guardian signature/request to administer medication.
Full explanation is printed in the handbook for each grade level & copies of the Medication order sheet are available at all school buildings.
All students will be provided an annual dental screen unless their parent/guardian chooses to opt their student out. Parent/Guardian may opt their student out of the annual screening by notifying one of the school nurses in writing.
Student Immunizations & Boosters
Student's Immunization records or updates to their records are due in the Health Office of the school they attend at enrollment or when boosters are received/required. Records are reviewed for KG, New Students & those who are due for required boosters annually. Exclusion dates will be set for students who do not provide proof of immunizations received & parents/guardians will be notified of needed Immunizations & exclusion dates.
