All Wellington High School juniors and seniors may attend After Prom even if they did not attend Prom. ONLY a parent or legal guardian can electronically sign this form. Parents must sign in using their own email, not a student account. All forms must be received by the Project Prom Committee before 12:00 pm (noon) on Friday, April 12th. One form per student must be completed, so if you have more than one junior or senior who is attending prom, please fill out a form for each student.
